By filling out this form, our organization is committed to be an active member of Empowering Piedmont. We are committed to the vision, mission, goals, objectives, and strategies that have been and/or will be decided by Empowering Piedmont. We are committed to the planning and collaboration that such groups undertake and understand that it will take time. We acknowledge the contributions and expectations of the other members of Empowering Piedmont

Benefits of membership include:

Resources, educational events, connection to other members, and access to strategic planning data. As general evidence of our commitment, we agree to do the following:

• Appoint a representative to attend regularly scheduled coalition meetings and other coalition activities.

• Authorize the representative to make decisions on our behalf. • Read minutes and reports to keep abreast of Empowering Piedmont decisions and activities.

• Disseminate relevant information to our organizational members and/or employees through listservs, websites, and newsletters. • Keep the coalition informed of our organizations’ related activities.

Commitment

Specifically, our organization will commit the following resources (e.g., access to relevant data, hosting refreshments, hosting events, meeting facilities, education, training, printing, technical assistance, expert advice, access to priority populations, workforce, IT services, and social media) to the coalition.
